I did not get my sign-in code

Each time you sign in, we email a 6-digit code to the address you entered. It usually arrives within a minute or two. If it has not come, try these in order.

1. Look in your spam or junk folder for an email from NDI Medical Forms.
2. Check the email address shown on the sign-in page. If it is misspelled, tap Use a different email address and type it again.
3. Tap Send a new code. The button is ready one minute after the last code was sent. If you end up with more than one email, use the code in the newest one. The earlier codes stop working.

A code works for 10 minutes. Type it into the same window where you entered your email address. If you closed that window, start again and we will send a new code.

If no code ever arrives, there may not be an account with that email address. For your privacy, the sign-in page never says whether an address has an account, so it will not tell you. You can create an account in a few minutes.

I used to sign in with a password

This site replaced our old forms site in September 2026, and it does not use passwords. Enter the email address you used before, and we will email you a code to sign in with. You will get a new code each time. Forms you signed on the old site are listed under Completed on your home page.

If you enter your email address and no code arrives, create a new account. It takes a few minutes.

I do not remember which email address I used

If you signed in with a username on our old site, you may not remember which email address you gave us. Call us and we can check. We will ask you a few questions to be sure it is you, then tell you which email address is on your account.

My email address has changed

If you can still sign in with your old address, open your profile and change your sign-in email there. You will be signed out, and we will send a code to the new address. Enter that code to finish the change. Until you do, your old address still works.

If you can no longer get email at your old address, create a new account with the address you use now. We cannot change the address on an account for you, because we have no way to be sure who is asking. Forms you already signed stay on file with us.

I cannot find my organization, or I picked the wrong one

Your organization is usually your fire department or ambulance company. If it is not in the list, please call or email us before you go any further, and tell us its name.

If you picked the wrong one, you can change it on your profile until you sign your first form. After that, call or email us and we will change it for you.

There are no forms on my home page

Your home page shows the forms for your organization, and it names the organization above the list. If the name is wrong, you can change it on your profile until you sign your first form.

If the organization is right, your forms may not be posted yet, or you may have signed them already. Signed forms are listed under Completed. If you were told to fill out a form and you do not see it, call or email us.

I need to stop and finish later

Your answers are saved each time you tap Next. You can also tap Save and finish later at any time. When you sign in again, the form is on your home page. Tap Continue and it opens where you left off.

I signed a form and need to change an answer

Once a form is signed, its answers cannot be changed on this site. Call or email us, and tell us which form and what needs to change. To see what you signed, open the form from Completed on your home page.

I am not sure my form went through

When you sign a form, we send a confirmation to your email address, and the form moves to Completed on your home page. If you see it there, we have it. You do not need to print anything. We print your forms for your screening.
