NDI, also known as Nassau Diagnostics, is a medical diagnostics company based on Long Island, New York. We provide physical screenings, blood draws, fitness tests, and drug screenings for fire departments, ambulance companies, and other organizations. Your organization arranges our visit, and you use this website, forms.nassaudiagnostics.com, to complete the forms we need before you are seen.
This website is run by N.Y. Diagnostics Management Corp, doing business as NDI. In this policy, "NDI", "we", and "us" mean that company. The policy explains what information the site collects about you, how we use it, who can see it, and the choices you have. It was last updated on September 12, 2026.

What we collect
We collect only what we need to identify you, prepare your forms, and get in touch with you.
Your account and contact details
- Your first, middle, and last name
- Your email address, which is also how you sign in
- Your mobile phone number, and a second phone number if you choose to give one
- Your date of birth, sex assigned at birth, and gender
- Your home address
- Your badge number, if your organization uses them
- The organization you belong to
- Whether you have agreed to receive text messages from us, and when you agreed
Your forms
- Your answers to every question on the forms your organization asks you to complete, including questions about your health and medical history
- Your initials and the date and time you sign each form
- The date you expect to attend, if you give one
Technical information
When you use the site, our servers record the internet address and browser type of your device along with the pages you visit. We keep these records to keep the site secure and to fix problems. We use one cookie, which keeps you signed in during your visit. We do not use advertising cookies or analytics services.
What we do not collect
You never create a password here. Each time you sign in we email you a one-time code that expires after ten minutes. We do not ask for payment information, a Social Security number, or insurance details, and we do not buy information about you from anyone else.
How we use your information
- To create your account, confirm your email address, and sign you in
- To show you the forms your organization has asked you to complete
- To print your completed forms for the technicians, nurses, and physicians who carry out your screening
- To send you the emails you need: your sign-in codes and a confirmation when you submit a form
- To send you text messages about your forms and visits, only if you have agreed to receive them
- To answer your questions when you contact us
- To keep the site secure, to prevent misuse, and to meet our legal obligations
We do not sell your information. We do not use it for advertising, and we do not share it with advertisers.
Who can see your information
Your information is seen only by the people who need it to carry out your screening.
- NDI staff who manage this site, review registrations, and print completed forms.
- The medical professionals who see you, such as the technicians, nurses, and physicians conducting your screening. They receive your printed forms.
- Your organization, only as the forms you sign describe. Each form explains what your organization receives from your screening, for example when a fitness-for-duty declaration states that a designated official may be told the result of a drug test. Your organization cannot read your answers through this site.
- Companies that help us run the site. Our email provider delivers sign-in codes and notices to your address, and our hosting provider stores the site's data on servers in the United States. They may use your information only to provide those services to us.
- When the law requires it, for example in response to a subpoena or court order, or when needed to protect someone's safety.
How we protect your information
Your connection to this site is encrypted. Sign-in codes work once and expire after ten minutes. Staff have their own sign-in, and each sees only what their role requires. Your completed forms are stored on our servers and printed for your screening; they are never posted anywhere public. No website can promise perfect security, so if a security problem ever affects your information we will tell you as New York law requires.
How long we keep it
We keep your account while your organization continues to arrange screenings with us, so you do not have to register again each year. We keep completed forms as part of your screening record for as long as the law and our agreements with your organization require. If you would like your account closed, contact us and we will close it, keeping only the records we are required to retain.
Your choices
- Your profile. Sign in and open your profile to update your name, address, phone numbers, or email address at any time.
- Your organization. You choose your organization after you first sign in and can change it from your profile until you have submitted a form. After that, contact us and we will change it for you.
- Text messages. Agreeing to text messages is optional. You can withdraw your agreement at any time by unchecking the box on your profile.
- Your forms. Once you sign a form it can no longer be changed on the site. If something on it is wrong, contact us before your screening and we will help you correct it.
- Your records. You may ask us for a copy of the information we hold about you, ask us to correct it, or ask us to delete it. Contact us using the details at the end of this page and we will respond within a reasonable time. We may have to keep records that the law or our agreements require us to keep.
Age
This site is for people in the organizations we serve. You must be at least 14 years old to register. If you are under 18, please read this policy with a parent or guardian.
Changes to this policy
If we change this policy we will post the new version here and update the date at the top. If a change affects how we use information you have already given us, we will also tell you on this site the next time you sign in.
