Terms of service

These terms cover your use of forms.nassaudiagnostics.com, the website where people at fire departments, ambulance companies, and other organizations complete the forms NDI needs before a screening. The site is run by N.Y. Diagnostics Management Corp, doing business as NDI, also known as Nassau Diagnostics. In these terms, "NDI", "we", and "us" mean that company, and "you" means the person using the site.

By creating an account or using the site you agree to these terms and to our privacy policy, which explains how we handle your information. If you do not agree, do not use the site; your organization can tell you how else to complete your forms. These terms were last updated on September 12, 2026.

What the site is for

Your organization arranges screenings with NDI and decides which forms you must complete and by when. This site lets you register, fill out those forms on your phone or computer, and sign them, so they are ready when you are seen. The site is not the screening itself. What happens at your screening, and what your organization receives from it, is described on the forms you sign and in your organization's arrangement with NDI.

Your account

• Register once, in your own name, with an email address that only you can read. There are no passwords here: each time you sign in we email a one-time code to that address, so anyone who can read your email can sign in as you. Keep your email account secure, and tell us if you lose access to it so we can update your address.
• Keep your details accurate and current. Your name, date of birth, address, and phone numbers are printed on your forms and used by the people carrying out your screening.
• You must be at least 14 years old to register.
• Use only your own account. Do not register for someone else, share a sign-in code, or let another person use your account. If you think someone else has used it, contact us right away.
• You may close your account at any time by contacting us. We keep the records we are required to keep, as the privacy policy explains.

Your forms

• Answer every question truthfully and completely, to the best of your knowledge. Your answers guide the medical professionals who see you. A false or incomplete answer can affect your screening and any decision your organization makes from it.
• Typing your initials on a form is your signature. You agree that signing this way has the same effect as signing the same form on paper, and that you are the person whose name appears on it.
• Once you sign a form it can no longer be changed on the site. If you notice a mistake, contact us before your screening and we will help you correct it.
• Complete your forms before your visit, by the date your organization or NDI gives you. A form that is not complete may mean you cannot be seen that day.
• We may update a form between visits. Forms you have already signed keep the wording you saw when you signed them.

What the site is not

Nothing on this site is medical advice. The forms collect information for your screening; they do not diagnose or treat anything, and reading or completing them does not make you a patient of NDI or of any physician who works with us. Questions about your own health belong with your doctor. If you have a medical emergency, call 911.

The site is not a way to reach us urgently. If you need to speak with someone at NDI, use the contact details at the end of this page.

Using the site properly

Use the site only to complete your own forms and manage your own account. Do not try to sign in to another person's account, to reach pages or data you have not been given access to, to test or attack the site's security, to copy the site or its forms with automated tools, or to send anything designed to harm the site or the people who use it. We may suspend or close an account that breaks these terms, and we may tell your organization when we do.

Messages from us

We email you the codes you need to sign in and a confirmation when you submit a form. These messages are part of using the site and cannot be turned off while your account is open. We send text messages only if you have agreed to receive them on your profile, and you can withdraw that agreement at any time. Your mobile carrier's standard message rates may apply.

Availability and changes

We work to keep the site available, but we cannot promise that it will always be reachable or free of errors, and we may take it down for maintenance. We may change the site, add or remove forms, or stop offering the site altogether. If we make a significant change to these terms, we will post the new version here, update the date at the top, and tell you on the site the next time you sign in. Using the site after a change means you accept the new terms.

Who owns what

The site, its design, and the forms on it belong to NDI or to the people who license them to us. You may use them only to complete your forms. Your answers are your information. By entering them you give us permission to store them, print them, and share them as the privacy policy and the forms you sign describe.

Our responsibility to you

We provide the site as it is, and we take reasonable care to keep it working and to protect your information. To the extent New York law allows, NDI is not responsible for losses that come from your use of the site or from being unable to use it, including a screening that could not go ahead because a form was not completed in time. Nothing in these terms limits any right you have under New York law that cannot be limited by agreement, and nothing in them changes the responsibilities NDI and your organization have agreed between themselves.

Law and disputes

New York law governs these terms. If a dispute about the site cannot be settled by talking with us, it will be heard in the state or federal courts located in New York. If any part of these terms cannot be enforced, the rest still applies.
